What is chronic pain?
Everyone knows what pain is, but in conventional medicine, there are two basic categories for pain that patients need to understand: acute and chronic pain. Acute pain may be mild or severe but it only lasts a short period of time. Pain that lasts only a moment or that is anticipated to resolve within a few weeks or a few months would be considered “acute” pain. Acute pain usually resolves and changes over time, giving patients a sense of control over the unpleasantness. But chronic pain is different, so it impacts patients’ quality of life in a more pronounced way.
In contrast, chronic pain may be either mild or severe, but it is ongoing. Chronic pain lasts longer than 6 months which means that the patient must learn to cope with it daily over the course of a long period of time. As such, chronic pain can have a debilitating impact on patients’ lives if it can’t be managed. Treatments are geared at relieving enough of the pain that patients can be comfortable. Examples of common chronic pain issues include:
● Back pain
● Headache
● Arthritis
● Cancer
● Nerve pain
● Fibromyalgia
Chronic pain can cause physical effects on the body that further intensify the negative experience associated with the pain including muscle tension, impaired mobility, lack of energy, and changes in appetite and digestion. Many individuals who suffer from chronic pain also experience depression, anxiety, anger, and fear of re-injury.
Who suffers from chronic pain?
Approximately 50 million Americans suffer from chronic pain including between 11% and 40% of all U.S. adults. Women and older individuals are more likely to experience chronic pain than men and younger adults. Chronic pain is one of the most common health problems that leads Americans to seek medical care in the U.S. High-impact chronic pain causes limitations in various areas of a patient’s life including work, recreation, self-care, and socialization.
What type of doctors treat pain management?
Pain Management Specialists are doctors who have specialized training in evaluating, diagnosing, and treating different types of pain. Though pain may be caused by surgery, nerve damage, injury, or metabolic diseases like diabetes, the most widely recognized types of pain include:
● Acute pain
● Chronic pain
● Cancer pain
● A combination of the above
Sometimes, pain has no underlying cause that can be identified, but Pain Management Specialists can help treat these conditions using different tools such as oral or topical medications, specialized injections, electrical therapies, physical therapy, psychological therapy, and more.
When shopping for a Pain Management Specialist, you should look for someone with experience and training working with your specific problem. Interview several doctors until you find one with whom you feel comfortable. Your doctor should have completed a fellowship in anesthesiology, neurology, physical medicine, or rehabilitation beyond their residency. Most Pain Management Specialists are board certified by the American Board of Anesthesiology.
What are common Chronic Pain Management Techniques?
Chronic pain negatively impacts quality of life and often, it’s difficult to treat chronic pain effectively so that patients can reliably find relief. But there are a number of treatment options available that include:
● Oral medications
● Topical medications
● Injection treatments
● Electrical therapies
● Physical therapy
● Acupuncture
● Hypnotherapy
● More…
Oral Medications
Oral medications include several categories of drugs that can be purchased either over-the-counter or with a prescription. Below are the most common categories of oral medications that are used to treat chronic pain:
● Opioids
Opioids are only available with a prescription and though they can be extremely effective at controlling pain, some of them are addictive. At higher doses, opioids don’t perform as effectively against nerve pain, but chronic, ever-present pain that lasts throughout the day and night may require the use of a long-acting opioid. Patients who take opioids may experience digestive upsets including constipation or nausea as well as drowsiness. And patients also need to be aware of the potential for addiction and consider other potential treatment options before taking opioids. As a general rule, opioids should only be used to treat chronic pain as a last resort. Below are the most commonly prescribed opioids for chronic pain:
❖ Hydromorphone (brand name: Dilaudid®)
❖ Codeine (brand name: Tylenol No. 3®)
❖ Oxycodone (brand names: OxyNeo®, Percocet®)
❖ Morphine (brand names: Kadian®, MS-Contin®, M-Eslon®, Statex®)
❖ Tramadol (brand names: Zytram®, Tridural®, Ralivia®)
● Nonsteroidal Anti-Inflammatory Drugs (NSAIDs)
NSAIDs are pain reducers that also thin the blood, decrease inflammation, and decrease fevers. Some NSAIDs are available over-the-counter, but they may still increase the risks of bleeding ulcers, heart attack, kidney disease, or gastrointestinal upsets. This type of drugs are a good choice for muscular and bone pain, but because they can have a negative effect on the kidneys, long-term use is usually discouraged.
❖ Aspirin
❖ Naproxen (brand name: Aleve)
❖ Ibuprofen (brand names: Motrin, Advil)
❖ Indomethacin (brand name: Indocin)
❖ Celecoxib (brand name: Celebrex)
❖ Etodolac
❖ Diflunisal
❖ Diclofenac (brand names: Cataflam, Cambia, Zorvolex, Zipsor, Voltaren-XR)
● Acetaminophen (brand name: Tylenol)
Acetaminophen performs many of the same functions as NSAIDs, but has less of an impact at reducing inflammation in the body. In high doses, acetaminophen can cause severe liver failure. Sometimes acetaminophen is combined with opioid medications. Taking an over-the-counter acetaminophen product with certain opioid meds that also contain acetaminophen can result in an overdose.
● Antidepressants
There are several different categories of antidepressants that are effective at diminishing pain. Tricyclic antidepressants have the strongest pain-relieving properties at relatively low doses. Patients must take them every day, not just when they’re experiencing pain in order to reap the benefits of this type of therapy. Side effects can include a feeling of sedation, dry mouth, blurry vision, nausea, orthostatic hypotension, weight gain, foggy thinking, constipation, heart arrhythmias, and problems having sexual intercourse. Below are some of the most common antidepressants that are prescribed for pain:
❖ Desipramine (brand name: Norpramin)
❖ Clomipramine (brand name: Anafranil)
❖ Amitriptyline
❖ Imipramine (brand name: Tofranil)
❖ Doxepin (brand name: Silenor)
❖ Nortriptyline (brand name: Pamelor)
❖ Protriptyline (brand name: Vivactil)
● Muscles relaxers
Muscle relaxants are best used to treat pain that’s caused by muscle spasm. They often cause drowsiness.
● Anticonvulsants
Anticonvulsants have been successfully used to treat certain types of nerve pain such as postherpetic neuralgia (caused by shingles) or diabetic neuropathy, but they must be taken daily and not on an “as needed” basis. Side effects include drowsiness and weight gain. Some of these medications can be hard on the kidneys, liver, and eyes. Some patients may experience a skin rash, confusion, nausea, vomiting, and digestive upsets as well as suicidal thoughts while taking anticonvulsants. Examples of common anticonvulsants used to treat chronic pain include:
❖ Carbamazepine
❖ Gabapentin
❖ Pregabalin
❖ Oxcarbazepine
Topical medications
Topical medications are treatments that can be applied to the skin as a cream, an ointment, or a patch. Topical meds are often placed over the area that feels pain. Medications that are used in the patch include substances such lidocaine or fentanyl. Some topical medication options can be purchased over-the-counter while others require a prescription. Common topical medications that are used to treat chronic pain include:
● Topical opioids
● Lidocaine
● Clonidine
● Topical NSAIDs
● Capsaicin
● Counterirritants/Rubefacients
Injection Treatments
Either superficial or invasive injection treatments can be done into or around the spine to provide relief from certain types of pain in the body. Epidurals can provide pain relief in the neck, arm, leg, or back. Superficial injections called trigger-point injections can be made directly into painful muscles and facet injections can be made into joints in the back or the neck to relieve burning pain in the arms or the legs due to Reflex Sympathetic Dystrophy or Complex Regional Pain Syndrome. The most common injection treatments that are used to treat chronic pain include:
❖ Facet Joint Injection
A facet joint injection provides relief of back and neck pain. They are always performed using x-ray. Soreness where the needle was inserted is a common side effect.
❖ Epidural Steroid Injection
An epidural steroid injection is administered in the neck and the back near an area of inflammation that is causing pain in the body. Typically epidural steroid injections are performed to alleviate pain in the back and leg or in the neck and the arm or hand. Doctors typically use x-ray guidance to perform the injections. Side effects including temporary numbness in the affected extremity, though there is a risk of ongoing numbness from this procedure.
❖ Lumbar Sympathetic Block
A lumbar sympathetic nerve block can be performed to treat Complex Region Pain Syndrome type I. It is administered in the lower back and it can cause back soreness and temporary numbness. Persistent numbness or weakness may also occur after a lumbar sympathetic block. This procedure is typically performed under x-ray guidance.
❖ Stellate Ganglion Block
A stellate ganglion block is usually performed either to diagnose Complex Regional Pain Syndrome in the hand or the arm, or to increase blood flow and treat pain in that area. Soreness, droopiness of the eyelids on the side of the body where the injection was administered, stuffy nose, and difficulty swallowing are common side effects from this procedure.
❖ Celiac Plexus Block
A celiac plexus block is used to relieve pain in the abdomen. Pain caused by cancer of the pancreas is a common problem that’s treated using a celiac plexus block. The injection is administered through the patient’s back.
Physical Therapy
In physical therapy, patients typically learn to do special exercises that stretch the muscles or the joints in a manner that relieves pain. Physical therapy also builds muscle strength in an effort to balance the action of the various muscles in the body and teach the patient new ways to perform daily activities and therefore avoid aggravating pain. Physical therapy retrains the central nervous system to desensitize it to pain signals coming from different areas of the body. Below are common therapeutic methods used by physical therapists to treat chronic pain:
● Exercise
● Patient Education
● Manual Therapies (including bodywork)
● Transcutaneous Electrical Nerve Stimulation (TENS) treatment
● Graded Motor Imagery
● Sensory Re-education
● Hot and Cold Therapy
Alternative and Holistic Treatments
Alternative and holistic pain treatments typically attempt to avoid or reduce the use of pharmaceuticals for pain management. Alternative treatments may include any of the following:
● Acupuncture
● Chiropractic
● Hypnotherapy
● Chiropractic
● Exercise
● Vitamins, Minerals, and Supplements
● Stress-reduction techniques
