For Medicare patients, we provide Chronic Disease Management (CDM) which is ongoing care and support for those with long-term conditions like diabetes, hypertension, heart disease, and COPD. The goal of CDM is to improve your quality of life, slow disease progression, and reduce hospitalizations by helping you manage your symptoms and stick to care plans.
Through Chronic Disease Management, we also offer Care Management (CCM) and Remote Patient Monitoring (RPM):

CCM gives you more personalized, continuous care with the coordination of healthcare services like regular check-ins with a care team, medicine management, and care plans to help you stay on track with your health goals.

RPM allows healthcare providers to track your vitals like blood pressure and glucose from your home, so you don’t have to go to a doctor visit in person. This helps the healthcare providers give timely treatment to lower your chances of needing to go to the emergency room.

New York State Emergency Rental Assistance Program (ERAP)
ELAP provides rent financial assistance. You are eligible if you
- Fell behind in rent since the COVID-19 pandemic began in March 2020 AND
- Qualified for unemployment benefits or have lost income and/or have an increase in expenses due to the COVID-19 pandemic; AND
- Have a monthly gross (before tax) household income at or below 80% of the Area Median Income (AMI).

Housing Counselors

Home Energy Assistance Program (HEAP)
HEAP is a program that helps low-income homeowners and renters pay for heating and cooling utility bills. You are eligible if
- Your household received a HEAP benefit during the current HEAP program year OR
- You are currently receiving Temporary Assistance (TA) or Supplemental Nutrition Assistance Program (SNAP).
- For cooling assistance, a household member must have a medical condition that is exacerbated by heat.

Empower. Assist. Care. (EAC) Network
If you live in Nassau county, the Empower. Assist. Care. (EAC) Network can help you access the following programs:

Lifeline
