How it Works
Assessment
We speak with you (and your family, if helpful) to understand your health, living situation, and goals.
Care Plan Creation
You’ll work with a dedicated care manager who will create a custom plan just for you.

No, your CDPAP services will remain as they are. This is a separate program also covered by Medicaid.
There is no change to your health coverage, this is a program covered by Medicaid and does not require a specific plan.
Your Care Manager at FreedomCare is there to find and connect you with additional resources that your health plan may not be able to do. We can help with social services, housing assistance, doctors’ referrals, and more.
The appointments are conducted in-person at the Patient’s home, over the phone or online via Zoom.
You do not have to have a particular health plan; we only require active Medicaid coverage.
You do not have to be enrolled with FreedomCare to receive Care Management services. You just need to have active Medicaid coverage and qualifying conditions and needs.
The first 2 appointments must be in person; the good news is the Care Manager will come to your home for the appointments. Appointments after that can be conducted either in-person or virtually.
