This program is also available in New York, Nevada, Missouri, Pennsylvania, Arizona, Georgia, Indiana, Colorado, Connecticut and Oregon.
Getting care you trust is simple as 1 – 2 – 3.
1. Live in Massachusetts
You must live in the state of Massachusetts.
2. Live with a Caregiver
You must live in the same home as the Caregiver (either your home or the home of the Caregiver).
3. Have Medicaid
Your Primary Care Physician will need to refer you to Adult Foster Care program, but don’t worry! We can help you!
