To qualify for HHP, you must live in Michigan, have Medicaid, and require help with daily activities like bathing, dressing, or mobility. You must be at least 18 years old and be able to direct your own care, or have someone you trust who can do this for you.
To qualify for HIDE-SNP, you must live in a HIDE-SNP approved area, have Medicaid & Medicare, and require help with daily activities. You must be at least 21 years of age.
Due to state regulations, having dual eligibility in Medicaid & Medicare could make you eligible for the HIDE-SNP program. For the Home Help program, you need Medicaid to qualify.
A caregiver must be at least 18 years old, eligible to work in the United States, and not excluded from the Medicaid program. Family members, including adult children or siblings, often qualify as long as they meet these basic requirements.
The Michigan Home Help Program, managed by the Department of Health and Human Services (MDHHS), helps people stay independent in their own homes instead of moving to a nursing facility. Through this program, family members, friends, or other trusted caregivers can be paid to provide care, easing financial stress while ensuring loved ones get safe, consistent support.
To qualify, the person receiving care must live in Michigan, be eligible for Medicaid, and need help with daily activities because of age, disability, or a medical condition. They’ll also need to complete an in-home needs assessment with an MDHHS specialist and have a physician’s statement confirming the medical necessity of care.
The MI Coordinated Health Plan serves people who qualify for both Medicare and Medicaid.
To qualify for the MI Coordinated Health Plan or HIDE-SNP, the person receiving care must live in a HIDE-SNP approved area, have Medicare Part A & Part B, Medicaid and need help with daily activities.
