If you work 130 hours or more per month, you can sign up for health insurance through Total Plan Concepts using the PHCS network.

Is there a premium?

Yes. Depending where you live in Colorado, your health plan will vary. Each plan has its brochure with plan information. If you are a Denver resident, your premium (or weekly payment) would be $44.17 per week. Click here to view your health plan brochure.

How do I sign up?

To sign up, complete the Group Benefit Form within 90 days of your start date and email it to [email protected] or fax it to (347)-230-5491. If you sign up, you will start receiving health insurance 90 days after your start date.

If you want to sign up for health insurance after 90 days of employment, you must have a qualifying life event or wait until open enrollment.

Can I add my child or dependents to my health care plan?

To add a child under 26 to your health insurance plan, it costs $21.51 per week. If you are a Denver resident, it costs $27.04 per week. They would be enrolled in the same medical plan as you have.

You can add your child(ren) or dependents to your health insurance plan during any of the following times:

  • within 90 days of your start date at FreedomCare (coverage will start 90 days after your start date)
  • if they have a qualifying life event
  • during open enrollment

Can I add my spouse to my health care plan?

You cannot add your spouse to your plan.

  1. When can I start using sick time?
    Right away! Colorado allows Medicaid caregivers to use accrued sick leave as soon as it’s earned—there’s no waiting period. 

 

  1. How does sick time accrue?
    You earn 1 hour of paid sick leave for every 30 hours worked. Whether you’re a full-time caregiver or working flexible hours, this ensures equitable access to time off. 

 

  1. How can I use this time?
    Caregivers can use their sick time in hourly increments. This flexibility is ideal for short appointments or recovery days. 

 

  1. How many hours can I use per year?
    You may use up to 48 hours per year. This state-mandated cap ensures coverage without overextension. 

 

  1. Can unused time roll over?
    Any unused time can roll over into the next calendar year, helping you plan for the unexpected. 

 

  1. Do I need a doctor’s note?
    Only if you’re out for 4 or more consecutive days. This policy keeps things simple for common illnesses. 

 

  1. Can I get paid for unused time?
    No, payouts for unused time are not permitted under Colorado law. 

 

  1. What qualifies for use?
    Approved reasons include personal illness, caring for a sick loved one, preventative care, and needs related to domestic violence or public emergencies. 

Overtime refers to any hours that you work over 40 hours in a week. You get paid time and one half per hour when you work over 40 hours in a week.
