  • Have Medicaid
Massachusetts-Medicaid-card
  • Live in Massachusetts
  • Need help with day-to-day activities
  • Able to direct your own care/services (if not, have an individual representative to appoint)
  • 16 years of age and older

  • Someone who is in good health, and is able to read & write
  • Someone who is not excluded from the Medicaid program
