3+ months of personal care experience (home or facility) within the last 3 years, or formal training (HHA/nursing school).
Cannot be the patient’s spouse, parent, son, son-in-law, daughter, or daughter-in-law.
Must live at a different address from the patient.

Patient Requirements:

Must be enrolled in a health plan that works with a participating PCA provider.
Once the caregiver and patient onboarding is complete, we require the authorization from the patient’s health plan to move forward with LHCSA.
