Step 2: Request Structured Family Care services through your case management agency
Your case management agency submits the request to begin personal care services and acts as the point of contact with Missouri Medicaid.

Step 3: Complete an assessment to determine your plan of care
A state approved assessment is completed to evaluate your daily care needs and determine which services and how many hours of care are appropriate. As part of this step, you may be asked to obtain medical records from your Primary Care Physician to support your care needs. These records help the assessor understand your health history and ensure the plan of care accurately reflects the support you need at home.

Step 4: Complete authorization and orientation with FreedomCare

Once the state approves care, you will complete the final authorization and onboarding process with FreedomCare so services can officially begin.

This step includes three parts:

  • Caregiver Enrollment
    During caregiver enrollment, our team collects all required documents and completes a background check. We guide you through each requirement so nothing feels confusing or overwhelming.
  • Orientation
    One of our Onboarding agents will schedule a call to complete orientation. During this call, we walk through how to submit hours for payment, review caregiving expectations, and answer any questions you may have. Many caregivers tell us this is when everything finally clicks.
  • Begin Services
    After orientation is complete and authorization is finalized, the caregiver can begin providing care and getting paid to support the patient at home.

Step 5: Attend your welcome call to confirm your official start of care date
The welcome call confirms final details, answers questions, and lets you know exactly when care and caregiver pay will start.
