If you work 130 hours or more per month, you can sign up for health insurance through Total Plan Concepts using the PHCS network.

Is there a premium?

Yes. Your premium (or weekly payment) for health insurance would be $44.17 per week.

How do I sign up?

To sign up, complete the Group Benefit Form within 90 days of your start date and email it to [email protected] or fax it to (347)-230-5491. If you sign up, you will start receiving health insurance 90 days after your start date.

If you want to sign up for health insurance after 90 days of employment, you must have a qualifying life event or wait until open enrollment.

Can I add my child to my health care plan?

To add a child under 26 to your health insurance plan, it costs $27.04 per week. They would be enrolled in the same medical plan as you have.

You can add your child(ren) to your health insurance plan during any of the following times:

  • within 90 days of your start date at FreedomCare (coverage will start 90 days after your start date)
  • if they have a qualifying life event
  • during open enrollment

Can I add my spouse to my health care plan?

You cannot add your spouse to your plan.

  1. When can I start using sick time?
    After 90 days of working as a Medicaid caregiver in Oregon.
  2. How does sick time accrue?
    You earn 1 hour of paid leave for every 30 hours worked, helping caregivers balance care responsibilities with their own well-being.
  3. How many hours can I use each year?
    A maximum of 56 hours per calendar year may be used for sick leave.
  4. Can unused time carry over?
    Yes. Any unused leave can carry over to the next year, offering peace of mind for future needs.
  5. Do I need a doctor’s note?
    Yes, if you’re out for more than 3 days in a row.
  6. Do I need to give advance notice?
    If possible, provide notice before taking time off. Oregon caregivers are expected to notify employers when they can.
  7. Can I cash out unused time?
    No, this is not permitted by Oregon state law.
  8. What qualifies for sick leave?
    The law allows caregivers to use sick time for illness, family care, medical appointments, mental health needs, or safety-related absences.
