What Immediate Need Home Care Means in New York

Immediate Need is a pathway within New York Medicaid for people who need personal care or home care services right away and have no informal supports to rely on in the meantime. Informal supports means family, friends, or neighbors who could safely provide care while a regular application is processed. If no one is able to fill that gap, the Immediate Need track exists so the person is not left without help.

The idea behind it is simple. A standard Medicaid home care application can take time, and some situations cannot wait. Someone comes home from the hospital and cannot bathe or prepare meals on their own. A parent’s condition changes suddenly and the family realizes they cannot keep up with the care safely. Immediate Need is meant to move faster than the usual timeline so care can start sooner.

It is administered through New York Medicaid, not through any single agency. That is an important distinction, and it shapes everything below, including what FreedomCare can and cannot do for you.

How the Immediate Need Process Works

The process runs through the local agency that handles Medicaid in your area. To request Immediate Need services, an applicant generally files a Medicaid application along with an attestation that they have an immediate need and no one available to provide care, plus a physician’s order describing the care the person requires. New York sets shorter timeframes for these requests than for a standard application, so the local agency reviews and decides more quickly.

The exact forms, documentation, and timelines are set and administered by the Medicaid agencies that handle these requests. Because those requirements can change and are applied by the agency reviewing your case, the right people to confirm current forms and deadlines are the offices listed in the next section, not us.

Who Decides Eligibility and Where to Apply

Eligibility for Immediate Need is determined by New York Medicaid, through the agency that handles your case. FreedomCare does not decide whether you qualify and does not process Immediate Need requests. For questions about eligibility, required documents, or how to apply, you should contact the office that administers Medicaid where you live:

  •     Your local Department of Social Services (DSS), if you live outside New York City.
  •     The Human Resources Administration (HRA), if you live in New York City.
  •     Your Medicaid Managed Care Plan, if you are already enrolled in one. Your plan can tell you how Immediate Need works within your coverage.

These are the offices that review Immediate Need requests, confirm what paperwork is needed, and determine eligibility for services. If you call and feel like you are getting nowhere, that is a common experience, and it is worth being persistent and asking specifically about the Immediate Need process by name. It is a defined pathway, and the staff who handle it can point you to the current forms.

Where FreedomCare Fits: Staffing Your Care After Approval

Here is where we come in. Once the Medicaid agency or your managed care plan reviews the request and authorizes home care services, you need an agency to actually provide the care. That is what FreedomCare does. We are a licensed home care services agency offering personal care aide (PCA) and home health aide (HHA) services across New York.

After your services are approved and authorized, we can begin coordinating care and staffing your case according to the plan of care that was approved. We do not change what was authorized or decide the level of care. We work within it, and we handle the parts that come after approval: matching a qualified aide to the case, onboarding, and the ongoing support that keeps care running.

To be clear about what we do not do, because families ask and it matters: we do not determine whether you qualify for Immediate Need, we do not complete or submit the application or paperwork for you, and we do not approve requests.  Those steps belong to the Medicaid agencies described above. If someone asks us about the Immediate Need process, we can share general information and point you to the right office. The decisions themselves are made by New York Medicaid.

Who Immediate Need Home Care Is For

This pathway tends to matter most for a few situations. A person coming home from a hospital or rehab stay who cannot manage daily tasks alone and has no one at home to help. An older adult whose needs increase suddenly, where the family cannot safely keep up. An adult with a disability or a chronic condition whose usual support falls through. In each case, the common thread is urgency plus the absence of someone who can step in during the wait.

If that describes what you are dealing with, Immediate Need is worth asking your local Medicaid office about directly. And once care is approved, we are ready to staff it.

How to Get Started
