FreedomCare heart logo Eligibility 

Caregiver Requirements: 

  • Must have worked with the patient for at least 3 months.
  • Cannot be the patient’s spouse, parent, son, son-in-law, daughter, or daughter-in-law.
  • Must live separately from the patient.
  • If you are not onboarded as a CDPAP case, you must have an active PCA/HHA certification.

Patient Requirements: 

  • Must provide consent to transition from CDPAP to LHCSA.
  • Authorization will be updated after the caregiver passes the ACD test and completes Onboarding.
  • The patient’s health plan must work with LHCSA (call us to check eligibility).
