If you work 130 hours or more per month, you can sign up for health insurance through Total Plan Concepts using the PHCS network.
Is there a premium?
Yes. Your premium (or weekly payment) for health insurance would be $44.17 per week.
How do I sign up?
To sign up, complete the Group Benefit Form within 90 days of your start date and email it to [email protected] or fax it to (347)-230-5491. If you sign up, you will start receiving health insurance 90 days after your start date.
If you want to sign up for health insurance after 90 days of employment, you must have a qualifying life event or wait until open enrollment.
Can I add my child to my health care plan?
To add a child under 26 to your health insurance plan, it costs an additional $27.04 per week. They would be enrolled in the same medical plan as you have.

Can I add my spouse to my health care plan?
You cannot add your spouse to your plan.
You can get up to 72 paid sick hours per year. Your sick hours start accruing right after you start working. You get 1 sick hour for every 30 hours you work, up to 72 hours per year. If you are sick and cannot make it to work, you may take 1 paid sick hour for every hour that you are scheduled to work that day as long as you have accrued enough sick hours.
You may begin using paid sick time 120 days after your first workday. To take your sick time, please contact the Caregiver Benefits team at 929-227-4392, or email at [email protected]
You cannot take more than 72 paid sick hours per year. If you don’t use your paid sick hours, you can roll over up to 72 hours of sick time to the next year for 1 year.
