If you work 30 hours per week or more, you can sign up for health insurance through Total Plan Concepts using the MagnaCare network.

Is there a premium?

Yes. Your premium (or weekly payment) for health insurance would be $44.17 per week.

How do I sign up?

To sign up, complete the Group Benefit Form within 90 days of your start date and email it to [email protected] or fax it to (347)-230-5491. If you sign up, you will start receiving health insurance 90 days after your start date.

If you want to sign up for health insurance after 90 days of employment, you must have a qualifying life event or wait until open enrollment.

Can I add my child to my health care plan?

To add a child under 26 to your health insurance plan, it costs $27.04 per week. They would be enrolled in the same medical plan as you have.

You can add your child(ren) to your health insurance plan during any of the following times:

  • within 90 days of your start date at FreedomCare (coverage will start 90 days after your start date)
  • if they have a qualifying life event
  • during open enrollment

Can I add my spouse to my health care plan?

You cannot add your spouse to your plan.

  1. When can I start using sick time?

You can start using sick time 120 days after your start date. 

  1. How do I earn sick time?

You earn 1 hour of paid sick time for every 30 hours worked. 

  1. How much sick time can I use each year?

You can use up to 72 hours of paid sick time per year. Any unused hours expire at the end of the following year. 

  1. Do I need to give notice to use my sick time?

If your time off is planned, you must give at least 7 days’ notice. If it’s unplanned, notify us as soon as possible.  

  1. Do I need a doctor’s note?

Yes, but only if you are out for three or more consecutive days. 

  1. Can I be paid for unused sick time?

No, unused sick time cannot be cashed out. 

  1. What can I use sick time for?

You can use it for your own or a family member’s: 

  • Illness, injury, or mental/physical health condition
  • Medical appointments or preventive care
  • Quarantine or exposure to contagious illness
  • School or workplace closure due to a public health emergency
  • Issues related to domestic violence or sexual assault (e.g. medical, legal, relocation, counseling)
  1. Are there other types of leave available?

Yes: 

  • Military Leave: You’re entitled to unpaid leave and job protection if you serve in the military (including training or drills). Advance notice is required.
  • Crime Victim Leave: You may take unpaid leave to attend court or testify if you’re a victim or a representative of a victim.
